Restoring Tone and Definition, Without Surgery
Most patients who come to us about facial ageing describe it in much the same way. They rarely point to a single line or a particular wrinkle. They say their face looks tired, or that something has softened that they cannot quite name.
There is a reason for that. Facial ageing is not one change but several happening quietly alongside one another. The skin gradually loses its firmness. Volume settles lower than it used to sit. And underneath both, the muscles that hold the face up slowly lose their tone.
Skincare and skin treatments work on the surface. Fillers replace volume. But until recently there was no non-surgical way to treat the muscle layer, even though it does much of the work of holding the face where it belongs.
EMFACE treats that layer. It uses gentle electrical stimulation to contract and re-tone the facial muscles that lift the face, while radiofrequency energy works on the skin above them at the same time. No needles, no incisions and no recovery time.
Dr. Tasneem describes it to patients as a gym session for the face, which is a fair description of how it works. Muscles that are exercised become stronger and hold their position better, and the muscles of the face are no different.
What Is EMFACE?
EMFACE is made by BTL and delivered through self-adhesive applicators placed on the face. Two things happen at once.
Muscle stimulation. The device produces contractions in specific facial muscles that are far stronger than anything you could achieve by moving your face yourself. Over four sessions, this restores tone to muscles that have weakened with age.
What matters here is which muscles are targeted. The face has muscles that lift, and muscles that pull down. As we age, the lifting muscles weaken while the ones pulling down keep working as they always did, and the balance tips. This is a large part of what makes a face look tired or heavy. EMFACE works on the lifting muscles only, which is why the result looks like a lift rather than a tightened, over-worked face.
Radiofrequency. At the same time, controlled heat is delivered to the skin. This stimulates the cells that produce collagen and elastin, firming the skin and improving texture and fine lines.
One thing worth being clear about. EMFACE does both, but not equally. Most of the lift comes from the muscle. The radiofrequency firms the skin, but it is gentler than the devices built specifically for tightening. If loose skin is your main concern rather than lost tone and definition, [Ultherapy], [HIFU], [Sofwave] or [Fotona skin tightening] will do more for you, and we will tell you so.
Which Areas of the Face Can EMFACE Treat?
We have all four applicators available for EMFACE, which means each area of the face can be treated properly rather than approximated with one.
Forehead and cheeks. These target the muscle that controls brow position, along with the main lifting muscles of the cheeks. Restoring tone here raises the brow, brings back definition through the midface and sharpens the jawline. The radiofrequency firms the skin over the same area.
Chin and under the chin. This applicator works on the muscle that supports the floor of the mouth and the area under the chin. When this muscle loses tone, the neckline softens, and no treatment aimed at fat alone will fix that. The radiofrequency runs at higher power here to reduce the fat layer as well. Together they improve both problems that create a double chin.
Eyes. The newest applicator, placed just outside the bony rim of the eye socket. Under-eye bags are not really a skin problem. They happen when the fat pad that sits below the eye pushes forward as the tissue holding it back weakens. This applicator tones the muscle that supports that fat pad, which treats the cause rather than the skin over it.
This is why it works where other treatments struggle. Tightening devices improve the skin around the eye but do nothing about the fat. Injections near the eye need real caution and are not suitable for everyone. In the manufacturer’s study of thirty patients treated over four weekly sessions, results at three months showed an average 1.4 ml reduction in under-eye bag volume, a 28% reduction in crow’s feet and a 64% reduction in dark circles. See also our page on [dark circles treatment].
Which applicators you need is decided at your consultation, based on your face.
How Do We Plan EMFACE Treatment?
We treat with EMFACE first, before making any decision about fillers. Only afterwards do we assess whether filler is actually needed, and for a good number of patients it turns out not to be.
The reason is simple. A face that has lost muscle tone often looks like it has lost volume, when what it has really lost is support. Filling that face adds weight without adding lift, and the result is the heavy, overfilled look that patients recognise immediately and do not want. Restoring the muscle first usually reduces how much filler is needed, and sometimes removes the need altogether.
Not everyone needs filler. Every patient receives an individualised treatment plan, built around what their face actually needs.
What to Expect During an EMFACE Session
Four sessions of twenty minutes per area, spaced about a week apart.
You lie down. No numbing cream and no anaesthetic are needed. Makeup is removed and jewellery taken off. The applicators are positioned according to the muscles being treated, and the intensity is increased gradually through the session, separately for the muscle stimulation and the radiofrequency, to whatever you are comfortable with.
You will feel your facial muscles contracting rhythmically, along with a steady warmth. Most patients find it strange for the first few minutes and unremarkable after that.
There is nothing to recover from. You can drive home, go back to work and put makeup on the same day.
How Many EMFACE Sessions Are Needed, and When Will I See Results?
Some patients notice their face feeling firmer during the course itself. The real changes come afterwards, as collagen rebuilds and muscle tone settles in. Results are usually at their best six to twelve weeks after the final session.
For the forehead and cheek treatment, the manufacturer’s studies report average improvements of around 37% in wrinkle reduction, 30% in muscle tone and 23% in lift after a full four-session course.
A note on those numbers. They come from studies funded by the company that makes the device. That is normal across aesthetic technology rather than a problem specific to EMFACE, but it is worth knowing when you read them. Independent research is more limited. We quote these figures because they are the published data, not as a promise of what you personally will get.
How long results last depends on your age, your starting muscle tone, your skin and your lifestyle. Many patients hold their result for about a year, with a single maintenance session every six to twelve months to keep it.
Who Is a Candidate for EMFACE?
EMFACE works well for adults with early to moderate facial ageing, where the main problem is loss of tone and definition rather than loose skin. It suits:
- Patients who dislike needles, or cannot have injectables
- Patients who want a natural result rather than an obviously treated one
- Patients who cannot take time off for recovery
- Patients losing definition through the cheeks and jawline
- Patients with under-eye bags who are not ready for surgery and are wary of injections near the eye
Who Should Not Have EMFACE?
EMFACE cannot be done if you have:
- An implanted electronic device, including a pacemaker, defibrillator, cochlear implant, nerve stimulator or insulin pump
- Metal implants, plates or screws in the treatment area, or facial piercings that cannot be removed
- Pregnancy or breastfeeding
- An active skin infection, open wound or severe acne in the treatment area
- Cancer, or a fever or acute illness at the time of treatment
- A neurological or muscular condition affecting the face
Needs assessment before treatment: recent filler or Botox in the area, previous threads, significant scarring, epilepsy, diabetes with reduced skin sensation, and metal dental work or braces.
Please tell us about every implant, device and previous treatment.
What Are the Side Effects of EMFACE?
EMFACE is one of the safer treatments we offer, but any treatment that delivers energy into tissue carries some risk, and you are entitled to know what it is.
Common, and settling within hours: redness, warmth or flushing, a feeling of tightness, and mild aching in the facial muscles, much like muscle soreness after exercise.
Less common: a temporary headache, tenderness, or a tingling sensation.
Rare: irritation from the adhesive applicator, or a burn if the radiofrequency is not properly controlled. This is why intensity is adjusted to your feedback throughout rather than set once and left alone.
Contact the clinic if you develop blistering, a burn, spreading redness, or any facial weakness or asymmetry that does not settle.
EMFACE vs. Botox, Fillers and Facelift Surgery
| EMFACE | Botox | Fillers | Facelift |
Type | Non-invasive, no needles | Injection | Injection | Surgery |
Works on | Muscle tone and skin | Relaxes muscle | Restores volume | Repositions tissue |
Effect on muscle | Strengthens | Weakens | None | None |
Downtime | None | Minimal | Minimal to a few days | Weeks |
Best for | Lost tone and definition | Lines from movement | Lost volume | Sagging and excess skin |
The key difference is direction. EMFACE strengthens facial muscles. Botox relaxes them. They are not alternatives, and they are often used on different parts of the same face. Fillers add volume, which EMFACE does not do. Each treats a different part of facial ageing, and most patients past forty have more than one.

Frequently Asked Questions
How many EMFACE sessions do I need?
Four sessions of twenty minutes, about a week apart. A maintenance session every six to twelve months helps keep the result.
Does EMFACE hurt?
No. You feel your facial muscles contracting and a warm sensation from the radiofrequency. No numbing cream is needed, and the intensity is adjusted to your comfort throughout.
Is there any downtime?
None. You can go back to work, drive and wear makeup straight away.
When will I see results?
Some firming may show during the course. Results are usually at their best six to twelve weeks after the last session, as collagen rebuilds and muscle tone settles.
Does EMFACE tighten skin?
To a degree, yes. The radiofrequency firms the skin and improves texture and fine lines. But EMFACE is mainly a muscle treatment, and most of the lift comes from the muscle. If loose skin is your main concern, Ultherapy, HIFU, Sofwave or Fotona will do more.
Can EMFACE treat under-eye bags and dark circles?
Yes. The eye applicator tones the muscle that supports the fat pad under the eye, treating the cause rather than just the skin. Significant fat or excess eyelid skin may still need surgery, which we will assess honestly.
Can EMFACE treat a double chin?
Yes. The chin applicator tones the muscle supporting the area under the chin while the radiofrequency reduces the fat layer, with no needles and no downtime.
Can EMFACE replace Botox or fillers?
No. EMFACE strengthens facial muscle, Botox relaxes it, and fillers add volume. They do different things. We usually recommend EMFACE first, then assess whether filler is needed at all.
Is EMFACE safe for brown skin?
Yes. It uses radiofrequency and muscle stimulation rather than light, so skin colour makes no difference and there is no risk of pigmentation.
Can I have EMFACE with a pacemaker or metal implant?
No. Implanted electronic devices and metal implants in the treatment area are absolute contraindications.
What does EMFACE cost in Karachi?
It depends on which applicators are used and how many areas are treated. This is confirmed at your consultation.
